How Public Contributors Helped Shape the ESTEEM Trial To Date: A Look Behind the Scenes

Public involvement can sound quite abstract. However, in the ESTEEM trial—which is exploring whether adding testosterone to standard Hormone Replacement Therapy (HRT) can reduce menopausal symptoms—it has been practical, visible and influential.

Over the past two years, public contributors have helped shape how the trial is designed and communicated, making it clearer, more inclusive and better grounded in everyday experience.

Who are the public partners on the project?

Public partners have been part of ESTEEM from the start. Two contributors with lived experience of menopause and HRT, Lisa M and Lisa N, are members of the Trial Management Group (TMG) and play an active role in decision-making in designing the study.

We have also worked closely with Fair Treatment for Women in Wales (FTWW) and with community groups, including the Talking Trials group in Cardiff and the Bargoed Parenting Group. Their insights have helped keep the study relevant and realistic.

Making the Trial More Inclusive – from Technology to Translation

Early on, public partners raised barriers linked to technology, resulting in the team introducing alternative ways of sharing information so contributors could stay fully involved.

Community groups highlighted the need for translated and simpler materials:  These suggestions directly shaped posters, FAQs and recruitment content. They encouraged us to look beyond GP surgeries, pointing to community centres and faith settings as important places to connect with people.

Building Strong Working Relationships

Public contributors helped improve how meetings worked. Changes to  meeting dates made it easier for public partners to attend regularly and have their voices heard.

In December 2024, public partners also co‑facilitated community events in Cardiff and Bargoed, helping to open up conversations about menopause and testosterone.

Learning Together and Reaching People Where They Are

Community engagement revealed gaps in awareness eg many women did not know that women naturally produce testosterone. Clearer explanations helped, while feedback about stigma around menopause symptoms in some communities shaped a more sensitive approach to communication.

Strengthening Governance – Clearer, Fairer, More Proportionate

In 2025, public contributors helped shape responses to regulators and strengthened clarity around consent, recruitment routes, GP communication and information‑sharing.

Transforming Our Recruitment Communications—Shorter, Clearer, Kinder

Public input led to visible changes in how ESTEEM communicates. Information is  shorter, clearer and less technical. Language is sensitive, and guidance for app‑based consent is easier to follow.

Contributors also suggested myth‑busting FAQs, audio information and short videos to support understanding.

The Impact: What Changed Because of Public Involvement?

Because of public involvement:

  • Trial procedures changed
  • Governance improved
  • Recruitment expanded into communities
  • Materials became clearer and more accessible
  • Acceptability of the study increased

Final Thoughts

Public involvement in ESTEEM has shaped how the trial works, communicates and makes decisions. It has helped create a more inclusive study that better reflects the people it aims to engage and recruit.

Gall cynnwys y cyhoedd swnio fel cysyniad haniaethol. Fodd bynnag, yn nhreial ESTEEM—sy’n trin a thrafod a all ychwanegu testosteron at Therapi Amnewid Hormonau (HRT) safonol leihau symptomau’r menopos—mae wedi bod yn ymarferol, yn weladwy ac yn ddylanwadol.

Dros y ddwy flynedd ddiwethaf, mae cyfranwyr cyhoeddus wedi helpu i lunio sut mae’r treial wedi’i gynllunio a’i gyfleu, gan ei wneud yn gliriach, yn fwy cynhwysol ac wedi’i seilio’n well ar brofiad bob dydd.

Pwy yw partneriaid cyhoeddus y prosiect?

Mae partneriaid cyhoeddus wedi bod yn rhan o ESTEEM ers y cychwyn cyntaf. Mae dau gyfrannwr sydd â phrofiad bywyd o’r menopos a HRT, Lisa M a Lisa N, yn aelodau o’r Grŵp Rheoli Treial ac yn chwarae rhan weithredol yn y broses o wneud penderfyniadau wrth ddylunio’r astudiaeth.

Rydyn ni hefyd wedi gweithio’n agos gyda Triniaeth Deg i Ferched Cymru (FTWW) a grwpiau cymunedol, gan gynnwys y grŵp Trafod Treialon yng Nghaerdydd a Grŵp Rhianta Bargoed. Mae eu syniadau wedi helpu i gadw’r astudiaeth yn berthnasol ac yn realistig.

Gwneud y treial yn fwy cynhwysol – o dechnoleg i gyfieithu

Ar y dechrau, nododd partneriaid cyhoeddus rwystrau yn gysylltiedig â thechnoleg, gan arwain at y tîm yn cyflwyno ffyrdd amgen o rannu gwybodaeth fel bod modd i gyfranwyr barhau i gymryd rhan lawn.

Tynnodd grwpiau cymunedol sylw at yr angen am ddeunyddiau wedi’u cyfieithu a deunyddiau symlach:  Arweiniodd yr awgrymiadau hyn yn uniongyrchol at lunio posteri, Cwestiynau Cyffredin a chynnwys recriwtio. Fe wnaethon nhw ein hannog i edrych y tu hwnt i feddygfeydd teulu, gan gyfeirio at ganolfannau cymunedol a lleoliadau ffydd yn lleoedd pwysig i gysylltu â phobl.

Adeiladu perthnasoedd gwaith cryf

Helpodd cyfranwyr cyhoeddus i wella sut roedd cyfarfodydd yn gweithio. Gwnaeth newidiadau i ddyddiadau cyfarfodydd hi’n haws i bartneriaid cyhoeddus fynd yn rheolaidd a dweud eu dweud.

Ym mis Rhagfyr 2024, cyd-hwylusodd partneriaid cyhoeddus ddigwyddiadau cymunedol yng Nghaerdydd a Bargoed hefyd, gan helpu i ddechrau sgyrsiau am y menopos a thestosteron.

Dysgu gyda’n gilydd a chyrraedd pobl l maen nhw

Fe wnaeth ymgysylltiad cymunedol ddatgelu bylchau mewn ymwybyddiaeth, er enghraifft nid oedd llawer o fenywod yn gwybod bod menywod yn cynhyrchu testosteron yn naturiol. Roedd esboniadau cliriach yn fuddiol, tra bod adborth am stigma ynghylch symptomau’r menopos mewn rhai cymunedau wedi llunio dull mwy sensitif o gyfathrebu.

Cryfhau llywodraethu – er mwyn ei wneud yn gliriach, yn decach ac yn fwy cymesur

Yn 2025, helpodd cyfranwyr cyhoeddus i lunio ymatebion i reoleiddwyr a gwella eglurder ynghylch cydsyniad, llwybrau recriwtio, cyfathrebu â meddygon teulu a rhannu gwybodaeth.

Trawsnewid ein dull cyfathrebu am recriwtio—byrrach, cliriach, mwy caredig

Arweiniodd mewnbwn y cyhoedd at newidiadau gweladwy yn y ffordd mae ESTEEM yn cyfathrebu. Mae’r wybodaeth yn fyrrach, yn gliriach ac yn llai technegol. Mae’r iaith yn sensitif, ac mae canllawiau ar roi cydsyniad ar apiau yn haws i’w dilyn.

Awgrymodd cyfranwyr hefyd gwestiynau cyffredin sy’n chwalu mythau, gwybodaeth glywedol a fideos byr i gefnogi dealltwriaeth.
