The World Health Organization has dedicated 2020 to nurses\nand midwives to create a \u2018once in a generation opportunity\u2019 to showcase the\ncritical role that the nursing and midwifery professions play in supporting\nglobal health. <\/p>\n\n\n\n

It chose the theme to coincide with the 200th anniversary of\nthe birth of Florence Nightingale. While Nightingale is known as the \u2018lady with\nthe lamp\u2019 for her work during the Crimean War, she is less well known for her\npioneering statistical work<\/a> with hospital and post-partum mortality data which\nlead to landmark public health reforms.  Her calls in the 1890\u2019s<\/a> for\nbetter use of routinely collected data, and frustration with policy-makers\u2019\nfailure to use it in decision making, will resonate with today\u2019s researchers. <\/p>\n\n\n\n

Nurses and midwives\u2019 role in research delivery<\/strong><\/h2>\n\n\n\n

As the largest group of health care professionals, nurses\nand midwives play a vital role in research delivery. The NIHR\n<\/a>are joining the celebrations, and see this year as being an opportunity to\ncelebrate the important work of nurses and midwives in research and highlight\nthe huge contribution that nurses and midwives play in improving health and\ncare services across a range of health and social care settings. <\/p>\n\n\n\n

Nurses and midwives (and other health and care\nprofessionals) play a vital role in the work of the Centre for Trials Research.\nMany of the clinical trials and complex studies that we lead rely on highly\nskilled research nurses and midwives, as well as nursing and midwifery\ncolleagues providing care for patients and members of the public, to\nsuccessfully deliver them. Their skills and experience are vital to ensure the\ndelivery of high-quality and safe participant-centred research across different\nclinical areas and care settings.<\/p>\n\n\n\n

Working collaboratively and in partnership with nurses\nand midwives<\/strong><\/h2>\n\n\n\n

Many of our trials are delivered in partnership with, and\nsupported by, research nurses and midwives from other organisations such as\nhealth boards and Health and Care Research Wales Research Delivery Staff in\nWales, and by the NIHR Clinical Research Networks in England. This includes our\nwork in primary and secondary care, and trials led by the Centre\u2019s cancer\ndivision.<\/p>\n\n\n\n

Some of our trials involve complex nurse-led clinical\nprocedures such as PRIMUS<\/a>\nwhich aims to create a clinical decision support tool to help GPs with the management\nof lower urinary tract symptoms in men. Others involve novel therapies for\nneurodegenerative conditions such as Huntington\u2019s disease (TRIDENT<\/a>). These trials also benefit from the Centre\u2019s\nresearch nurses who play a key role in managing and supporting these complex and\npotentially challenging trials. We also contribute to work that aims to improve\ncare delivery, such as exploring person-centred care in care homes, and\nevaluating interventions which aim to improve people\u2019s health and wellbeing\nsuch as social prescribing.<\/p>\n\n\n\n

Midwives also play an important role in a number of the\nCentre\u2019s trials, including a current trial to evaluate the safety of\nwaterbirths (POOL<\/a>)\nwhich is led by Professor Julia Sanders who is a Consultant Midwife and Professor\nof Clinical Nursing and Midwifery at Cardiff University and a long-term\ncollaborator with the Centre.<\/p>\n\n\n\n

Nurses as core members of the Centre\u2019s academic staff<\/strong><\/h2>\n\n\n\n

Unusually for a Clinical Trials Unit, we have nurses as\nmembers of the Centre for Trials Research academic staff. This means that we\nhave been able to support trials that require significant and\/or specialised\nresearch nurse support, such as PRIMUS and TRIDENT and our recently completed PRINCESS<\/a>\ntrial of a probiotic in care homes. This was a randomised controlled trial\nwhich could be considered challenging to deliver in a non-NHS setting with\nolder frail people, 70% of who have cognitive impairment affecting their\nability to provide informed consent. Having Research Associates who are nurses\nmeant that we could design the trial to be feasible to deliver in close\npartnership with care homes, and design and deliver training to research nurses\nfrom other networks\/organisations who were supporting the trial in other\nlocations. We have also shared the lessons we learnt more widely, recently publishing\nit as a Research\nMethods Case<\/a>.<\/p>\n\n\n\n

Moving from delivering research to developing researchers<\/strong><\/h2>\n\n\n\n

Non-medical health professionals are increasingly at the\nforefront of world-leading research, and the Centre supports a number of nurses\nand midwives developing their research careers. This is both through the Research\nDesign and Conduct Service<\/a> (RDCS SE Wales) we host which supports health\nand social care professionals to develop high quality applications for funding,\nand supporting the career development of our research staff and collaborators who\nare nurses and midwives through funding applications, publications, conference\npresentations etc.<\/p>\n\n\n\n

I have benefited from the Centre\u2019s support to develop my programme\nof research<\/a> into the ethical, legal, and practical issues encountered when\ninvolving adults who lack capacity to consent in research, which was recently\nrecognised at the Wales Nurse of the Year Awards. <\/p>\n\n\n\n

The findings from this novel area of research has in turn enabled\nme to support the trials we conduct which include populations who may have\nimpaired capacity to consent, including PRINCESS and others being conducted in\nemergency settings. <\/p>\n\n\n\n

A call to action!<\/strong><\/h2>\n\n\n\n

However, a recent\nreport<\/a> from the Council of Deans identified the challenges of recruiting and\nretaining an academic nursing, midwifery and allied health workforce. They\nexpressed concerns about the lack of academic career pathways in healthcare\ndisciplines, and the importance of increasing research capacity. The NIHR has\nalso identified the need to strengthen the research voice and influence of\nnurses and midwives in health and social care settings.<\/p>\n\n\n\n

The International Year of the Nurse and Midwife provides a\ngolden opportunity to highlight the important contribution from nurses and\nmidwives towards delivering and leading research in the Centre for Trials\nResearch and beyond.
